Posted 12 day(s) ago
Reference: 2444940
Job summary
We are seeking a compassionate, motivated and skilled Senior Healthcare Assistant / Nursing Associate to join our Frailty and Community Outreach Team within East & Central Brighton Primary Care Network.
Working as part of an integrated multidisciplinary team, the postholder will primarily support our Long-Term Conditions (LTC) workstream, delivering structured LTC reviews and routine clinical monitoring to housebound patients. The role will also contribute to the wider team's delivery of proactive, person-centred care to adults living with frailty, multiple long-term conditions and complex health or social needs.
This is an exciting opportunity to take on a varied and rewarding clinical role, working with a degree of autonomy within agreed competencies and under the guidance of registered clinicians. The postholder will undertake delegated clinical assessments, routine health monitoring and follow-up interventions, supporting the early identification of deterioration and helping patients remain safe and independent at home.
Working closely with GPs, nurses, allied health professionals and wider community services, the postholder will contribute to coordinated, preventative care, helping to improve health outcomes, reduce avoidable hospital admissions and address health inequalities.
Please note that this role is not eligible for Skilled Worker visa sponsorship. Applicants must have an existing right to work in the UK.
Main duties of the job
About us
East & Central Brighton Primary Care Network (PCN) is a collaborative network of nine GP practices, serving over 70,000 patients across some of Brighton's most diverse communities. As a Community Interest Company, we are committed to improving population health, reducing health inequalities and delivering accessible, high-quality care to those who need it most.
Our multidisciplinary teams bring together nurses, pharmacists, physiotherapists, occupational therapists, mental health practitioners and social prescribing professionals to provide proactive, personalised and coordinated care. We work closely with primary care, community services and voluntary organisations to address both the clinical and wider social factors affecting health.
Our Frailty and Outreach Team supports adults living with frailty, multiple long-term conditions and complex health needs, with a particular focus on housebound patients and those who experience barriers to accessing healthcare. Through comprehensive assessment, long-term condition management and early intervention, we aim to prevent avoidable deterioration, promote independence and support people to remain well in their own homes.

What is a DBS Check?
A DBS check is a background check that reveals any criminal convictions, cautions, reprimands, or warnings. The level of check required depends on the role:
Standard DBS: Checks for spent and unspent convictions, cautions, reprimands, and final warnings.
Enhanced DBS: Includes the above, plus any additional information held by local police thats considered relevant to the role.
Enhanced with Barred Lists: Includes all of the above, plus checks against the childrens and/or adults barred lists.
Who Needs a DBS Check
Most roles involving direct patient contact, working with children or vulnerable adults, or access to sensitive information will require a DBS check. The job advert and recruitment team will let you know if this applies to your role.
The Costs for a DBS
At Manchester University NHS Foundation Trust (MFT), roles that require a DBS check are subject to a cost recovery policy. This means that the cost of submitting and processing a DBS application if you are successful and join us, will be deducted from your salary within the first three months of employment. If, however, you withdraw from a conditional offer after the DBS process has begun, you will still be liable for the cost incurred.
