Job summary
Are you a compassionate and organised individual with excellent communication skills? Join Botesdale Health Centre as a care navigator and become a vital part of our dedicated team, ensuring our patients receive the right care, at the right time. This position is offered on a part-time basis for 17 hours per week, working the following days:
Wednesday 8:30 to 18:00 (1 hr lunch break)
Friday 8:30 to 18:00 (1 hr lunch break)
At Botesdale Health Centre, we pride ourselves on providing high-quality medical care in a friendly, courteous, and professional manner. As a care navigator, you will be at the forefront of our patient services, working across various touchpoints to guide and support our patients through their healthcare journey.
Main duties of the job
Patient Engagement: Serve as the first point of contact for patients, dealing with enquiries both on the phone and at the reception desk, providing a high-quality and responsive customer care service.
Referral Management: Process all referrals sent from GPs, efficiently utilising the NHS e-Referral Service (e-RS). This includes submitting urgent referrals, such as two-week-wait (2WW) referrals, recording e-RS references in clinical records, and tracking their progress to ensure timely patient care.
Triage Hub Operations: Play a crucial role within our Triage Hub, reviewing online consultation submissions, assessing urgency (categorising as emergency, urgent, routine, self-care/advice, or signpost), and identifying any red-flag keywords or safeguarding indicators.
Patient Communication & Coordination: Contact patients to explain triage outcomes and confirm care plans. Coordinate with the team in the triage hub to book appointments based on urgency, modality, & clinician, ensuring patient agreement and providing clear next steps and safety-netting advice.
Information Governance & Confidentiality: Adhere strictly to patient confidentiality at all times, only accessing patient data necessary for your role using your own login, & never discussing patient information in public areas. Report any suspected unauthorised access or confidentiality breaches immediately.
Safety & Escalation: Identify potentially serious problems & escalate concerns to the appropriate clinician without delay.
About us
At Botesdale Health Centre, you will be part of a supportive and forward-thinking team dedicated to making a real difference in our community. We offer a rewarding role where your skills will be highly valued, contributing directly to positive patient outcomes.
We are an equal opportunities employer and welcome applications from all backgrounds in line with the Equality Act 2010.
We are committed to safeguarding and promoting the welfare of children and vulnerable adults. All appointments are subject to appropriate pre-employment checks, including DBS clearance.

Job summary
Burlington Primary Care is looking to welcome a nursing associate to their team on a full or part-time basis.
This is a substantive clinical role, not a support post. You will carry your own caseload of patients within a clear supervision structure, contributing directly to chronic disease management, preventative health, and our immunisation and screening programmes. We are specifically looking for a nursing associate with competence, or a strong commitment to developing competence, in cervical screening (smear taking) and child immunisations. Both are core to what we do, and you will have the support and training to deliver them with confidence.
Patients are allocated through our GP-led recall and triage system. You will always have a registered nurse or clinician available for escalation, and your supervision will be structured and documented, not ad hoc.
Main duties of the job
- Run your own appointment clinics within your scope of practice, including chronic disease reviews for conditions such as diabetes, hypertension, COPD, and asthma.
- Deliver NHS Health Checks independently.
- Carry out cervical screening (smear taking) -- competency required or active training underway.
- Administer childhood and adult immunisations within supervised clinics and seasonal vaccination programmes, under a PGD or as directed by the supervising clinician.
- Perform venepuncture, ECGs, BP monitoring, urinalysis, and basic wound care.
- Support health promotion and lifestyle advice, including brief interventions and signposting.
- Contribute to recall and care planning for patients with long-term conditions.
- Maintain accurate, timely records on our clinical system (EMIS).
- Work collaboratively across the MDT, contributing to team meetings and quality improvement.
About us
Burlington Primary Care is a busy, forward-thinking practice in Ipswich, with a genuine commitment to patient-centred, preventative care. We run a growing multidisciplinary team that includes GPs, practice nurses, healthcare assistants, pharmacists, and paramedics. We believe the best care comes from teams that work well together, support each other, and keep learning. That is the environment you would be joining.
What we offer:
- NHS Pension Scheme.
- Generous annual leave entitlement.
- Structured clinical supervision with a named supervisor.
- Funded CPD and training, including smear competency and immunisation programmes where needed.
- A real MDT, including GPs, nurses, HCAs, pharmacists, paramedics, who all talk to each other!
- Flexible working arrangements where possible.
- Active involvement in service development and quality improvement.

Job summary
A Care Navigator is a very special person! They are the first port of call for patients and they respond to requests for GP appointments, made by patients either on the phone or in person. With training and support, they will triage the patient non-clinically, and signpost to the appropriate service or person, which may not necessarily be the GP. They will work in a reception hub alongside their colleagues (a team of 12) or on front desk reception.
We are looking for someone who is flexible enough to cover early starts (8am) and late finishes (6.30pm) across the week working around 30 hours per week including a Monday, Tuesday and Thursday at least.
Salary starts at £13.00 per hour with an increase after you pass probation to £13.50 per hour.
If you have applied previously please do not apply again.
Main duties of the job
This is a challenging but very satisfying role.
Patients rely on our care navigators to listen, advise and point them in the right direction. Often, patients themselves do not know what they need in terms of their health and it is up to a trained navigator to support them. Our team prides itself on the care and customer service we offer. Our access to appointments is excellent and prompt, but we rely on our navigators to ensure that the right person is seen at the right time by the right clinician (not necessarily the doctor!) This takes tact and diplomacy as well as patience.
About us
East Trees Health Centre is based in inner city Bristol. We are a training practice with a list size of 15000 patients.
Our practice team comprises of 4 partners, 7 salaried GPs, a paramedic, nurse practitioner, physio, podiatrist, dietitians, care co-ordinator, care navigators, administrators, nurses and pharmacy team. We are a very friendly, close knit team and have regular staff and team meetings to ensure communication is consistent.

Director Strategic Commissioning (Client Groups/Community Based Care)
NHS South East London Integrated Care Board View all jobs
Posted 16 day(s) ago
Reference: 2434955
Job summary
We are looking for an exceptional Director of Strategic Commissioning to join South East and South West London ICBs working together as an Integrated Care Board cluster. This role will be pivotal for the clustered ICBs as they refocus on strategic commissioning for a population of 3.8 million people with a commissioning budget of £4.75 billion for South East London and £4.3 billion for South West London.
The successful candidate must have the experience and capability to lead and inspire others to seize the opportunities of a period of transformation for ICBs. They will need the determination and discipline to make sure strategic and operational plans address local need, drive out inequality, deliver value for money and secure improved outcomes for local residents across our boroughs. In doing so they will have the vision and ambition to realise the potential of neighbourhood working, population health management and digital technology, and will align the commissioning process to deliver the Governments three strategic shifts: from treatment to prevention, from hospital to community, and from analogue to digital.
We would love to hear from candidates with commissioning expertise, a proven track record of successful strategic leadership, and who are excited by the possibilities of this complex and challenging role across the second largest ICB footprint in the country.
Main duties of the job
Commissioning end to end care pathways, inclusive of care pathway redesign and service specification.
Leading, convening and coordinating a significant part of the ICB's strategic and operational planning process as a key member of the Leadership team in the Commissioning Directorate.
Securing market development and management and utilisation management approaches to deliver on the commissioning portfolio.
Leading and overseeing the contracting and payment mechanisms to secure delivery.
Effective monitoring and management of operational delivery and performance.
Ensuring the effective development of strategic commissioning capabilities within the Directorate and wider organisation.
About us
In 2025, the boards of NHS South East London ICB and NHS South West London ICB took the decision to enter into a clustering arrangement. This means that we share a chair, chief executive officer and an executive team, but we remain as separate, statutory organisations.
This also means that the ICBs will share some teams, who will be working across both organisations. Some teams will also be working across both geographical areas and integrated care systems.
This is a period of change for the NHS. The Government's 10 Year Health Plan presents an ambitious vision for a future NHS, with emphasis on the three 'shifts' - from hospital to community, from analogue to digital, and from cure to prevention - all underpinning the new neighbourhood health service.
ICBs are also changing, with a clear focus on strategic commissioning and delivering neighbourhood working. This is an exciting time to work within our ICBs. We currently have the opportunity both to shape the future of our clustered organisations, and to be part of a radical transformation of care for the local people and communities across south London.
And while our two ICBs are different in some ways, improving lives for the people we serve is at the core of what we are both here to do.
Please note as you can only be employed by one ICB, this job description refers to 'the 'ICB', but your role may cover both ICB organisations, and / or geographical areas.
